Partial Dentures
Unlike full replacement full dentures which have inbuilt cross arch or bilateral balance, partial dentures suffer with a reduction in stability were the opposing arch of natural teeth is insufficient.This lack of replacement denture teeth results in vertical movements. This instability becomes more evident and when eating and cutting food as well as talking possibly to a lesser degree..
Where Insufficient balance is present (whether bilaterally or anteriorposteriorally) the underlying bone can receive varying degrees of pressure through impact. As well as not being ideal for the acrylic base longevity, more importantly the impact factor create shock waves that travel through the bone and cause it to shrink or resorb. The denture is then uncontrolable and requires additional denture commercial fixatives.
Patients often remark that a new partial denture requires frequent relines or refits and blame the manufacturer when in actual fact the partial denture has been placed in an unstabile situation in the first place.
The Advice to potential patients is: the inclusion of the opposing partial denture is essential and not to make more money out of you but to:-
1.to improve the longevity of the fit.
2.reduce bone resorption under the denture (saddle) biting areas.
3.Prevent the denture from fracturing.
4.Reduce twisting motions stressing motions on the supporting natural remaining teeth which in many cases are clasped(wired).
5.Prevent the gums and supporting bone around the natural teeth damage and potentially early supporting root loss.
